Membership Forms

The following forms are now available for download:

• Alumni Chapter Registration Form (rev. 2023). This form is for registering an alumni chapter for the first time with the ISC office.
• Collegiate Chapter Registration Form (rev. 2023). This form is for registering a collegiate chapter for the first time with the ISC office.
• Membership Annual Dues Update (Renewal). This form is for updating membership information and annual dues.
• Letter of Intent (LOI) for Chapter Affiliation Transfer . This form is for requesting chapter affiliation to another chapter.
• Financial Assistance Request Form. This form is for members who will be requesting financial assistance with the ISC office.
